Job Description


Primary Responsibilities**

Follows the plan of care to provide safe, competent care to the client as well as the laws of the Commonwealth of Pennsylvania in conjunction with the Pennsylvania Dept. of Health and the Office of Long Term Living.

Helps the client to maintain good personal hygiene and assists in maintaining a healthful, safe environment.

Plans and prepares nutritious meals, markets when instructed to do so by Care Plan.

Assists the client with ambulation as per the Care Plan.

Assists any therapy personnel as needed with rehabilitative processes.

Encourages the patient to become as independent as possible to the Care Plan.

Attempts to promote patient's mental alertness through involvement in activities of interest.

Gives simple emotional and psychological support to the client and other members of the household and establishes a relation with the client and family which transmit trust, and confidentiality.

Reports any change in client's mental or physical condition or in the home situation to the UILS Office Staff.

Performs routines housekeeping tasks such as laundry, dusting, sweeping, vacuuming, mopping floors, kitchen care, bathroom care, changing bed linens, and removing trash as related to a safe and comfortable environment for the patient, as instructed by the UILS Office Staff.

Prepares a visit report promptly and incorporates the same in the clinical record weekly.

Confirms on a weekly basis, the scheduling of shifts so that other necessary shifts by staff members can be coordinated.

Works with personnel of other community agencies involved in the client's care as directed by the Care Plan.

Attends in-service as required.

Comply with all policies and procedures established in the UILS Handbook.

Must accurately clock time of arrival and departure through EVV software.
