DESCRIPTION OF WORK.
1.1. The contractor shall provide Mobilization and Demobilization Medical screening and individual/soldier readiness processing in support of the Soldier Medical Processing and CONUS Replacement Center services for Irwin Army Community Hospital (IACH), Fort Riley, KS. These services include providing required immunizations, drawing appropriate bodily fluids, processing and turn-in collection of specimen screening and filing medical records for medical conditions, determining soldier fitness for duty (FFD), identifying disqualifying medical conditions for deployment, identifying conditions requiring further medical consulting, and completing IACH IMSU, computer entry and administrative requirements.
1.2. This is a non-personnel services contract and is not intended to create an employee relationship between the Government and the Contractor. The performance of the contractor and staff is independent and evaluated based on the performance measures established in the contract. The contracting officer and military officers/personnel provide day-to-day leadership, technical guidance and support as necessary to execute the terms in the contract.
1.3. The Non-Commissioned Officer In Charge (NCOIC)/Officer In Charge (OIC) will determine where work place is located during SRP vs. non-SPR process.
PERFORMANCE MEASURES.
2.1. Outcome. The contractor will provide qualified personnel who will fulfill the requirement to provide Mobilization and Demobilization Support/Soldier Medical Processing services at a standard, enabling the Medical Treatment Facility (MTF) to provide such services in a safe, efficient, Cost effective manner while still maintaining a high level of quality patient care.
2.2. Standards. Acceptable measures include: accurate and Timely completion rate-98% of the individual/soldier readiness processing (accurately identifying soldiers medically qualified and disqualified from mobilization, or demobilization, accurately identifying medical conditions requiring further medical consultation); employee turnover rate-30% per year; substantiated Patient complaints-maximum 2 per year per provider; and substantiated installation/MTF complaints. Other performance evaluation factors that will be monitored but that are not quantified by numerical measurements include but are not limited to: prescribing medications that are not on hospital formulary; contractor providing personnel exceeding the minimum qualification standards; no inappropriate prescriptions; patient customer service comments; provider and contractor relationship with hospital staff/government contracting personnel; compliance with installation and hospital policy and procedures.
2.3. How Measured. Administrative and Clinic supervisory personnel will monitor provider performance through government information systems and records, patient records, customer service information, contractor reports, and time sheets.
QUALIFICATIONS. Contactor provider personnel performing under this contract shall be able to read, write, and speak English fluently. They shall be rested, physically and mentally capable of performing the duties required under this contract.
3.1. Health care practitioners and medical support personnel, be qualified by academic and practical education to provide patient services as required to meet the performance standards and operating procedures as outlined in the Required Specialties Attachment. Changes in procedural and/or documentation changes required by higher headquarters or legal mandates or decisions would be implemented without impact to the soldier, MTF or installation.
3.2. Have training and are licensed to perform administrative and clinical procedures mandated and/or implied to fulfill the Mobilization and Demobilization Support/Individual/Soldier Medical Processing service requirements.
3.3. Possess and maintain required certification appropriate for their field of work. IACH reserves the right to refuse the services of contract staff based on lack of certification, credentials or background checks. The IACH Commander has the authority to revoke or otherwise restrict the JD privileges of any contract staff member; this action does not release the contractor. From the responsibility of performing the work required by this contract. The contractor shall obtain all paperwork, letters of reference, approval, work-history, etc. required by the IACH credentialing committee. This shall be done IAW all requirements in Army Regulations 40-66 and 40-68 and IACH regulations.
3.4. Healthcare practitioners and providers must possess and maintain at all time a current Drug Enforcement Administration (DEA) registration number.
3.5. All medical support staff shall have and maintain certification in basic cardiac life support (BCLS), physicians’ physician extenders and nurse practitioners shall have and maintain certification in advanced cardiac life support (ACLS)
3.6. All contractor staff must meet all security and background checks as determined by the installation and Medical facility. All information related to the individual/soldier readiness processing, the workload requirements, mission completion, or data is considered official government information and may not be provided outside the government. Any violation is punishable under the federal law. This also includes the release of medical information under the Health Insurance Portability and Accountability Act.
3.7. All contractor staff must have the following immunizations prior to starting work under this contract: yellow fever, small pox, influenza, and proof of annual TB test and Hepatitis A&B.
3.8. All contractor staff shall complete the IACH required annual training. All contractor staff must be able to type and have computer experience and must obtain an Army Knowledge Online (AKO) account to access government computer systems. IACH will provide required orientation and training, including computer training for the CHCS, MEDPROS and MEDBASE computer systems and other systems as deemed appropriate by the installation or medical facility. Contractor will be asked to replace anyone who fails to adequately learn the computer systems at IACH within 2 weeks of training. Progress measurements of skill level per program will be periodically monitored and may be used as basis for personnel replacement. Contractor employees must also participate in on-site training for automation and soldier/individual readiness processing improvements and lessons learned.
3.9. Contractor shall provide qualified individuals within their specialty:
3.9.1. Optometrist must have a current License granted by the recognized licensing agency of a State, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands and 2 years clinical experience. Graduates of foreign medical schools will be required to possess both a medical license and other certification by the ECFMG or 5th Pathway.
3.9.2. Optometry and Hearing Technician must have certificate of training and 1 year experience.
PERFORMANCE.
4.1. HOURS OF PERFORMANCE. The services provided under this contract shall be on a 40-hour per
week basis. The MTF Commander shall establish the routine operating hours of the clinic, normally Monday through Friday, 7:30(a.m) – 4:30(p.m), excluding federal holidays. Although, operational hours can fluctuate due to workload and mission requirements. Services can be scheduled an 8-hour minimum or 12 hour maximum workday and can be scheduled on evenings, weekends and holidays if necessary.
4.1.1. Optometrist. Hours worked beyond the normal 80 hours in a two-week period shall be pre-approved by the COR. These hours are subject to the normal hourly bill rate as established in the contract, or offer the Optometrist compensable time off as agreed upon by the COR, Contractor and Contract Optometrist.
4.1.1.1. On-Call Services. No on-call services will be required by the contract Optometrist.
4.1.2. Ancillary Personnel. Ancillary Personnel shall not work more than 40 hours per week without being subject to overtime rates, although, any hours worked past 40 hours will be subject to overtime rates as established in the contract. However, the MTF Commander has the option to adjust the contract employee’s work hours during the scheduled workweek to offset overtime hours. As an example, if an employee works 2 additional hours on Monday, the government may allow the employee to take 2 hours off on Wednesday to offset the overtime, and this must occur during the same workweek. Permission to substitute time off in lieu of paid overtime must be obtained by the contractor and contract employee. If work is required on a holiday, the contract employee may be given another day off with regular pay or be given special holiday pay as incorporated into the contract.
4.2. The contractor shall provide Mobilization and Demobilization Support/Soldier Medical Processing primarily in at the Soldier Readiness Processing site, IACH, Fort Riley, Kansas. Upon a 24-hour notice, contractor shall provide medical exams at the same location as a part of the SRP process or at the SRP facility, Fort Riley, Kansas. Upon 24-hour notice, the contractor shall provide routine health care in a facility of the USA MEDDAC, Fort Riley, as scheduled and or provided by the installation, medical facility or designee; however, this contract covers usage of the contractor staff in all environments. Hours can vary as dictated by the mission to medically screen all soldiers scheduled to complete SRP processing on that day or as directed by the medical facility. Contractor staff shall maintain at all times a neat, well-groomed appearance and at all times shall comply with installation and medical facility dress code.
4.3. A projected schedule of work may be available to the contractor by the 15th day of the previous month; however, this is tentative and may be adjusted due to mobilization/ demobilization requirements as directed by the installation and/or medical facility. Schedule changes required by higher headquarters, installation, and medical facility to ensure mission completion shall not be the basis for any change or modification to the cost of this contract. SRP workload is projected and available within 48 hours of performance of work. The Contractor is expected to have sufficient staff on site to mobilize 30 per hour and demobilize 20 per hour until the daily mission is completed. The NCOIC/OIC will also be on-site. Staff must be on site for the scheduled start time of operations.
4.4. The contractor shall provide someone to interface, coordinate and communicate with participating installation and medical facility representatives as determined by the installation and/or medical facility. The government will provide liaison for coordinating medical consultations on soldiers temporarily disqualified from activation or release from active service.
4.5. The contractor shall provide all services, including medical briefings, data entry and reporting into medical information systems, review of forms, workload accounting and reporting, drawing bodily fluids, specimen collection, specimen turn-in and waste and biohazard disposal at SRP cite or IACH. Immunizations, optometry screens, optical fabrication submissions, forecasting and requesting all day-to-day operational requirements, mental health screens, data collection, performance reports, and daily ad hoc data collection and reporting requirements, in accordance with the standards set in military regulations and as referenced by the IACH SRP SOP document. Military regulations and policies are subject to change.
4.6. All contractor employees shall abide by all installation, medical facility and legal requirements and regulations concerning communication of data. All medical records, medical information, and reports remain the property of the government.
4.7. Unless otherwise specified, all financial, statistical, personal and/or technical data which is furnished, produced, or otherwise available to the contractor during the performance of this contract are considered U.S Government confidential business information and property. This information shall not be used for purposes other than performance of work under this contract.
4.8. The Government shall monitor the contractor's performance under this contract using the QA&I procedures established by IACH and AR 40-68. Additionally, the contractor's performance is subject to scheduled and unscheduled review by the QA&I Committee defined in IACH's quality assurance plan and AR 40-68.
CONDUCT.
5.1. The contract employees shall transmit their bi-weekly timesheet signed by the contractor's program manager to their prime contractor.
5.2. The contract employee shall abide by IACH regulations and requirements concerning the nature of limited privileged communication between patients and health care providers as may be necessary for security and personnel reliability program. They shall also abide by IACH regulations concerning the confidentiality of patient records, as embodied by federal statutes including the Privacy Act of 1974 -HIPPA. All regulations referenced are available for review at IACH. All medical records and reports will remain the property of the government.
5.3. The inspection and acceptance point for all services rendered under this contract will be by the SRP NCOIC. The performance by the contract employee, the quality of services rendered, and any documentation or written material in support of same, shall be subject to continuous inspection, surveillance and review for acceptance by the CO, COR and/or designated representative. Any services rendered by the contract employee to patients or interaction with IACH/military/civilian personnel deemed unprofessional/threatening/ dangerous by the NCOIC will be considered as grounds for immediate removal of the contract employee. Quality Assurance procedures established by IACH shall be used for continuous monitoring. Services provided by the contract employee shall be reviewed and rated in accordance with IACH by-laws and JCAHO standards.
CONFLICT OF INTEREST.
6.1. Contractor personnel shall not, while performing services under this contract, advise, recommend, or suggest to persons eligible to receive medical care at government expense that such persons should receive care from the contractor or contractor's personnel at any place other that IACH.
6.2. Contract personnel are not prohibited by reason of their employment under this contract from conducting private practice if there is no conflict with the performance of services under this contract.
6.3. Contractor personnel shall make no use of any government facility or other government property in conducting a private practice.
6.4. Confidentially of Information: Unless otherwise specified, all financial, statistical, personal, and/or technical data which is furnished, produced or otherwise available to the contractor during the performance of this contract are considered confidential business information and shall not be used for purposes other than performance of work under this contract. None of the above information shall be released by the contractor without prior written consent of the government.
OPTOMETRY (EYE) TECHNICIAN.
SCOPE OF WORK
The contractor shall provide the services of an Eye Technician for the Eye, Ear, Nose and. Throat (EENT) Department, Irwin Army Community Hospital (IACH), Fort Riley, Kansas during the non-SRP process. During SRP, the Eye Technician will be located at the SRP site.
SPECIFIC TASKS. The eye technician shall perform the following tasks:

PMCS equipment for proper functioning
Convert positive/negative prescriptions
Obtain prescription from lenses
Order military/frame of choice spectacles
Eye dilations
Perform visual fields, topography, photography, autorefraction & tonometry
